CME Post Test, Answer Form, and Activity Evaluation In order to receive a statement of credit, participants must successfully complete the CME post-test answer form and activity evaluation after reviewing the journal supplement. Participants will have two opportunities to pass the post test with a score of 80% or higher. This educational activity is offered as both a journal supplement with the January/February 2004 issue of The Journal of Supportive Oncology and the April 2004 issue of Oncology Special Edition. Please submit from only one activity for AMA PRA credit. Dual certificates are not available. Online Option: Click the "online evaluation" button below, complete the user information form, and submit your CME post-test answer form and activity evaluation electronically by January 31, 2005. Tests will be graded and results displayed instantaneously. Participants will receive an electronic, printable statement of credit immediately after successfully completing the post test. Mail or Fax Option: Click the "mail or fax option" button below to download the post test and answer form in Acrobat PDF format. Answer all post-test questions and activity evaluation questions and complete the name and address section. A statement of credit will be mailed to you within 6 to 8 weeks.
